PARENT - GUARDIAN EXPLICIT CONSENT DECLARATION FORM

As Gen Yapı Tasarım Proje Ve Uygulama Tarım Kimya Eğitim Eğitim San. Ve Tic. A.Ş. (Antalya Toplum Koleji), which acts as the data controller under Law No. 6698 on the Protection of Personal Data, and as the parent/guardian of our student, some of your personal data is processed accordingly. A significant portion of your personal data is processed within the framework of the exceptions stipulated in Article 5/2 of the Law on the Protection of Personal Data and the relevant laws specified. The necessary information has been provided to you in this regard. Because your special personal data contained in (former) identification documents and health data cannot be considered within the scope of the exceptions under the Personal Data Protection Law, they are special personal data for which explicit consent must be obtained for processing or sharing. Your personal data is processed for the purposes of student registration, the conduct of activities in accordance with legislation, and emergency management processes, with your explicit consent for applications and notifications. Your personal data is shared with the Ministry of National Education to the extent necessary to achieve the above-mentioned purpose, ensuring data security and taking the necessary measures by our company. I declare that the personal data and special categories of personal data I have shared with Antalya Community College are accurate and that I consent to their processing, storage, and sharing by Antalya Community College for the purposes specified above. I have read and understand the Disclosure Text on the Protection and Processing of Personal Data and my rights.

STUDENT EXPLICIT CONSENT DECLARATION FORM

Having the title of data controller under the Personal Data Protection Law No. 6698 Gen Yapı Design Project and Application Agriculture Chemistry Education and Training Industry and Trade Inc. (Antalya Community College)As our student, some of your personal data is processed in accordance with the principles stipulated in Article 4(2) of the Personal Data Protection Law. A significant portion of your personal data is processed within the framework of legal compliance stipulated in Articles 5 and 6 of the Personal Data Protection Law. You have been provided with the necessary information on this matter.

The personal data listed below is personal data and special personal data for which explicit consent must be obtained for the processing or sharing of which cannot be considered legally compliant under the Personal Data Protection Law. We would like to remind you that you may withdraw your consent.

Since your religious information and health data included in your (old) identity documents cannot be considered within the scope of exceptions in the KVKK, they are your special personal data for which explicit consent must be obtained for processing or sharing.

Your health data is processed to carry out activities in accordance with legislation, to carry out guidance activities, to be used in emergency processes, and to manage situations requiring urgent medical intervention.

Your health data is shared with healthcare institutions and authorized institutions and organizations to the extent necessary to achieve the above-mentioned purposes, by ensuring your data security and taking the necessary measures by our company.

While there is no purpose in processing the religious information contained in your (old) identity documents, it is indirectly shared with the individuals and institutions with whom your old identity document should be shared. However, we also inform you that these are the primary practices in processes where old identity documents are replaced with new ones and religious phrases are rendered illegible through scribbling.

Your visual and audio personal data may be shared on the official websites and social media accounts of the Ministry's central and provincial organizations, including the educational institution where my student is studying, for the purpose of promoting the activities and events organized within the scope of education and training processes and sharing them with the public;
